Normally, shanked lug nuts do not have a flange:

You then need to get appropriate washers to use with the lugs.
Sounds like you have a set of proprietary lugs; Appliance and Toyota OEM shanked lugs are not interchangeable with aftermarket lugs. Perhaps you have a set of one of those. Just go to your local wheel shop and get the appropriate length shanked lugs.
As for threading in studs, I don't know it that is exactly safe to do. Most studs are not rated for strength like bolts are. But, OTOH, if that is what you have to do, then there is not much choice...
